STAFF OFFICER REPORT FORM

Please complete and submit this form to file a  Monthly Staff   Officer's Report or for a members to express a concern, make a recommendation or report on an issue.  Monthly Staff Officer reports should be submitted prior to 1400 hours the day of the monthly meeting so they will be available for the evening meeting.   When the form is submitted, it sends an email and also formats a page suitable for printing.  Please print a copy for your records.
